Job summary

Elara Caring is hiring a Hospice Registered Nurse Triage (Part-Time) to provide patient care in home health settings. You will work every other weekend (Sat/Sun), 8 AM–8 PM, delivering high-quality care within the patient’s home and coordinating with the care team.

Requires RN or LPN licensure, a nursing degree, and 3 years RN experience with 1 year in supervisory roles. Hospice/home health experience preferred; base pay is $35.00–$38.00 per hour, depending on qualifications.

Qualifications

• Graduate of an accredited School of Nursing.
• Current State License as a Registered Nurse or Licensed Practical Nurse required.
• 3 years of RN experience in appropriate clinical care setting and 1 year of supervisory experience.
• Experience in hospice or home health environment preferred.

Responsibilities

• Provide patient care across the continuum in home health setting.
• Deliver 1:1 patient care and coordinate with the care team to ensure right care at the right time.
• Report to Branch Director and maintain high standards of clinical care.

What is Required?

• Graduate of an accredited School of Nursing

The base rate for this position is$35.00-$38.00 hourly and is based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
