Trinity Health At Home – Grand Haven seeks a Hospice Registered Nurse Case Manager to deliver skilled nursing care in patients’ homes within the Muskegon, MI coverage area. You will create individualized care plans, administer medications, and educate patients and families to support comfort and quality of life.

The role features a strong team environment with nurse-led leadership, meaningful 1:1 patient care, and opportunities for clinical growth.

Competenze

• Active RN license in Michigan.
• 1+ years of clinical nursing experience (Hospice preferred).
• Strong communication, assessment, and organizational skills.
• Compassionate, dependable, and mission-minded.
• Valid driver’s license and reliable transportation required.
