Job summary

Concho Hearts Hospice is seeking a compassionate PRN Registered Nurse (RN) to provide skilled nursing visits in Kilgore, TX and surrounding communities. You will assess patients, manage symptoms, and educate families while traveling to homes and care facilities.

This role offers flexible scheduling, mileage reimbursement, and meaningful one-on-one patient relationships. The RN will collaborate with an interdisciplinary team, coordinate medications and equipment, and document in the EMR.

Qualifications

• Active Texas RN license or qualifying NLC license.
• Hospice/home health or acute care experience preferred.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and time-management abilities.

Responsibilities

• Provide skilled nursing visits to hospice patients in homes and facilities.
• Perform comprehensive nursing assessments and symptom management.
• Educate patients and families on medications and care goals.
• Collaborate with interdisciplinary team to coordinate care.
• Complete accurate EMR documentation in a timely manner.
• Respond to patient needs and urgent requests per schedule.
• Support admissions, visits, and plan-of-care adjustments as needed.

We're looking for a compassionate and dependable PRN Registered Nurse (RN) to join our team serving Kilgore, Texas, and surrounding communities.

What You'll Do:

• Provide skilled nursing visits to hospice patients in their homes, assisted living communities, and skilled nursing facilities.
• Complete comprehensive nursing assessments and evaluate patient needs.
• Provide pain management, symptom control, and comfort-focused care.
• Recognize changes in patient condition and communicate with physicians and the interdisciplinary team.
• Educate patients and families about medications, disease progression, comfort measures, and end-of-life care.
• Assist with admissions, routine visits, or urgent patient needs based on scheduling and qualifications.
• Coordinate medications, medical equipment, supplies, and other patient needs.
• Collaborate with RN Case Managers, LVNs, Hospice Aides, Social Workers, Chaplains, and physicians.
• Complete, accurate, and timely documentation in the EMR.
• Provide compassionate emotional support and guidance to patients and families.

What We're Looking For:

• Active Texas Registered Nurse (RN) license or qualifying Nurse Licensure Compact (NLC) license.
• Previous nursing experience in hospice, home health, skilled nursing, or acute care preferred.
• Hospice nursing experience is a plus!
• Strong clinical assessment and critical-thinking skills.
• Excellent communication, organization, and time-management skills.
• Ability to work independently while collaborating with the clinical team.
• Compassionate, dependable, and committed to exceptional patient care.
• Current CPR certification.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to travel throughout Kilgore and surrounding communities.
