Job summary

Concho Hearts Hospice is seeking a compassionate Registered Nurse (RN) to join our Kilgore team as a Hospice Admissions Nurse. This full-time role focuses on guiding families through the transition into hospice care and coordinating admission activities.

You will conduct home, hospital, and facility visits, perform comprehensive nursing assessments, educate families about hospice services and medications, and ensure smooth documentation in the EMR while collaborating with the interdisciplinary

Qualifications

• Active Texas RN license.
• Hospice or home health experience preferred.
• CPR certification required.

Responsibilities

• Complete hospice admission visits in patients' homes, hospitals, facilities, and other care settings.
• Perform comprehensive nursing assessments.
• Educate patients and families about hospice services, medications, equipment, and what to expect.
• Review and complete admission documentation and required consents.
• Collaborate with physicians and the hospice interdisciplinary team.
• Coordinate medications, DME, supplies, and other immediate patient needs.
• Help ensure a smooth transition into hospice services.
• Communicate important patient information to the RN Case Manager and clinical team.
• Complete accurate and timely documentation in the EMR.
• Provide reassurance and compassionate support to patients and families throughout the admission process.

The decision to begin hospice care can be overwhelming for patients and families. At Concho Hearts Hospice, our Admissions Nurses help make that transition a little easier.

We're looking for a compassionate and confident Registered Nurse (RN) to join our Kilgore team as a Hospice Admissions Nurse.

This is a great opportunity for an RN who enjoys patient interaction, education, clinical assessment, and helping families understand what hospice care can provide.

What You'll Do:

• Complete hospice admission visits in patients' homes, hospitals, facilities, and other care settings.

What We're Looking For:

• Active Texas Registered Nurse (RN) license.
• Strong nursing assessment and communication skills.
• Hospice or home health experience preferred.
• Comfortable educating patients and families about end-of-life care.
• Organized and able to manage admission documentation efficiently.
• Compassionate, dependable, and confident working independently.
• Reliable transportation, valid driver's license, and current auto insurance.
• Current CPR certification.
