Job summary

Companion Hospice is seeking a compassionate RN Case Manager in Oklahoma. You will guide patients and families through end-of-life care with autonomy, clinical expertise, and empathetic leadership.

Qualifications

• Current Oklahoma RN license.
• 1 year of medical/surgical, oncology, home health, or hospice experience recommended.
• Strong knowledge of pain and symptom management in terminally ill patients.
• Excellent communication, organization, and problem-solving skills.
• Valid driver’s license, reliable transportation, and current auto insurance.

Responsibilities

• Develop and manage individualized Plans of Care with patients, families, physicians, and the IDG.
• Coordinate hospice services and ensure seamless care across settings.
• Provide expert pain and symptom management.
• Educate patients and families on Medicare Hospice Benefit and hands-on care.
• Supervise Home Health Aides and deliver high-quality care.
• Complete timely documentation and participate in admissions.
