Job summary

Otterbein SeniorLife is seeking a Hospice RN Case Manager in Ohio to manage a primary caseload and coordinate comprehensive end-of-life care. The role follows Hospice Medicare Conditions of Participation and Ohio licensure rules, working with families and an interdisciplinary team.

The ideal candidate will have 2–4 years of hospice care and strong nursing leadership, with a compassionate approach to patient comfort and family support.

Qualifications

• Bachelor's degree in nursing required or preferred.
• Valid Ohio RN license.
• 2–4 years direct patient care in hospice and case management preferred.

Responsibilities

Shift: Full-Time 8a-4:30p Monday-Friday (on call and weekend rotation)

Responsibilities

• Otterbein is seeking an enthusiastic and professional Hospice RN Case Manager
• This role is responsible for primary caseload.
• This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.

Qualifications

• Education : Bachelor's degree in nursing preferred. Graduate of approved nursing program
• Licensure/Certification: Valid Ohio Registered Nurse (RN) license
• Experience: A minimum of 2 - 4 years' direct patient care experience in hospice and Case Management experience preferred
• Experience: Experience with end-of-life diagnosis preferred
