Otterbein SeniorLife is seeking a Hospice RN Case Manager to oversee a primary caseload and deliver skilled nursing care within the Hospice Medicare framework. The role supports patients, families, and caregivers, aligning with our mission-driven, person-centered care philosophy.

Full-time position in Ohio with on-call and weekend rotation. RN license in Ohio and hospice/kindred experience are preferred to join our compassionate team of Partners.

Formación

Shift

Full-Time 8a-4:30p Monday-Friday (on call and weekend rotation)

Responsibilities

• This role is responsible for primary caseload.
• This position provides skilled professional nursing care to patients, their families and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws and agency policies and procedures.

Qualifications

• Education: Bachelor's degree in nursing preferred. Graduate of approved nursing program
• Licensure/Certification: Valid Ohio Registered Nurse (RN) license
• Experience: A minimum of 2 - 4 years' direct patient care experience in hospice and Case Management experience preferred
• Experience: Experience with end-of-life diagnosis preferred
