Job summary

PruittHealth Hospice is seeking a caring Registered Nurse Case Manager (RN) to join our hospice team in Georgia. You will evaluate patients, coordinate plan of care, and ensure timely documentation while delivering compassionate home or facility-based hospice services.

Qualifications

• Current, active RN licensure in state of practice.
• Hospice or home health nursing experience preferred.

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS

• Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
