Job summary

ViaQuest Hospice in West Lafayette, IN is seeking a compassionate Certified Nursing Assistant to provide in-home or facility-based patient care under the supervision of an RN. You will support daily living activities, monitor health status, and help with emotional well-being.

The role requires travel within the service area, reliable transportation, and a commitment to high-quality, person-centered care. Comprehensive training and a benefits package accompany full-time employment.

Qualifications

• Current CNA certification.
• Age 18 or older.
• Valid driver's license, car insurance, and reliable transportation.
• Willingness to travel within the service area.
• Ability to push, lift, or carry up to 50 lbs; able to crouch, kneel, balance.

Responsibilities

• Perform duties per plan of care.
• Communicate patient status updates to supervising RN.
• Maintain a safe patient environment.
• Provide emotional support to patients and families.
• Maintain current patient records and progress notes.
• Collaborate with the interdisciplinary care team.
