Interim HealthCare Hospice is seeking a devoted Hospice Registered Nurse (RN) in New Orleans, LA. This full-time role focuses on compassionate, home-based end-of-life care and collaborating with an interdisciplinary team to honor patient goals.

Qualifications

• Graduate of an accredited nursing program and active RN license in Louisiana.
• Minimum of 1 year of home hospice nursing experience.
• CPR certification.
• Valid Louisiana driver's license, auto insurance and transportation.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing specialty, strong communication skills and compassion.

Responsibilities

• Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life.
• Work as part of an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments; develop individualized care plans.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
• Maintain accurate documentation in accordance with organizational and regulatory standards.
• Participate in quality initiatives that elevate hospice care for the community.

• Consistent & Predictable Scheduling: Full-time: Monday–Friday, 8:00 AM–5:00 PM. On-call responsibilities as needed. PRN RN opportunities also available

As a Hospice Registered Nurse, you will:

• Provide home-based care to patients with a prognosis ofsix months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments. Develop individualized care plans that honor each patient’s goals
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals
• Maintain accurate documentation in accordance with organizational and regulatory standards
• Participate in quality initiatives that elevate hospice care for the community

A few must-haves for Hospice Registered Nurses:
