Interim HealthCare Hospice is seeking a devoted Hospice Registered Nurse (RN) in New Orleans, LA. This full-time role focuses on compassionate, home-based end-of-life care and collaborating with an interdisciplinary team to honor patient goals.

Qualifications

Graduate of an accredited nursing program and active RN license in Louisiana.
Minimum of 1 year of home hospice nursing experience.
CPR certification.
Valid Louisiana driver's license, auto insurance and transportation.
Knowledge of state and federal home health regulations.
Clinical proficiency in nursing specialty, strong communication skills and compassion.

Responsibilities

Provide home-based care to patients with a prognosis of six months or less to live, focused on quality of life.
Work as part of an interdisciplinary team to carry out the patient’s plan of care.
Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments; develop individualized care plans.
Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals.
Maintain accurate documentation in accordance with organizational and regulatory standards.
Participate in quality initiatives that elevate hospice care for the community.
