Job summary

Interim HealthCare Inc. in Greenville, SC is seeking a compassionate Hospice Registered Nurse (RN) to lead patient-centered end-of-life care with empathy and clinical excellence.

This role focuses on comprehensive assessment, care planning with the IDG, and coordinating services to support patients and families. You will provide education, administer medications per physician orders, and advocate for dignity and comfort.

Qualifications

• Graduate of an accredited RN program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.
• Hospice experience preferred.

Responsibilities

• Completes an initial/comprehensive assessment of the patient/family to determine hospice needs.
• Completes ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family.
• Initiates and develops a plan of care in collaboration with the IDG, the patient/family, and the physician.
• Makes revisions to the IDG plan of care as the patient’s/family’s status and needs change.
• Coordinates care among IDG members and other providers to ensure continuous assessment and delivery of hospice services.
• Evaluates and coordinates the need for services provided to the patient/family in accordance with the IDG plan of care.

WhatYou’llDo

As a Hospice Registered Nurse (RN), you will:

• Completes an initial/comprehensive assessment of the patient/family to determine hospice needs.
• Completes ongoing assessments of the patient’s physical status and the psycho-social status of the patient/family.
• Initiates and develops a plan of care in collaboration with the IDG, the patient/family, and the physician.
• Makes necessary revisions to the IDG plan of care as the patient’s/family’s status and needs change.
• Primarily responsible for the coordination of care among IDG members and other providers external to the IDG to ensure the continuous assessment of the patient/family needs, as well as the implementation of the IDG plan of care to guide the delivery of services and achieve the goals of hospice care.
• Evaluates and coordinates the need for services provided to the patient/family in accordance with the IDG plan of care.
• Administers medications and treatments as prescribed by the physician in accordance with nursing standards of practice.
• Provides education and interventions to support the effective management of the patient’s pain and other physical symptoms as well as assesses the patient/family’s response to intervention and treatment
