Job summary

A leading home care provider in South Carolina is seeking a Hospice Registered Nurse to deliver compassionate care during end-of-life. The role encompasses initial and ongoing patient assessments, and development of care plans in collaboration with the care team. Candidates need to possess an accredited RN license, a year of nursing experience, with hospice experience preferred. Qualifications

• Must have a valid Registered Nurse (RN) license.
• Experience in hospice care is a plus.
• Ability to perform comprehensive patient assessments.

Responsibilities

• Completes an initial/comprehensive assessment of the patient/family to determine hospice needs.
• Initiates and develops a plan of care in collaboration with the IDG, the patient/family and the physician.
• Completes ongoing assessments of the patient’s physical status and psychosocial status.

Skills

Graduate of an accredited Registered Nurse (RN) program

Active, unrestricted Registered Nurse (RN) license in the State of Georgia or compact multi-state license

Twelve months of accumulated experience as a registered nurse within the last five years

One year of hospice care experience preferred

Education

Accredited Registered Nurse (RN) degree

Salary Info: $80,000.00 - $85,000.00 per Yearly

Competitive pay: Up to $85,000 / annually based on experience

• Completes an initial/comprehensive assessment of the patient/family to determine hospice needs which includes a history of current and previous illness(es), as well as a physical assessment of the patient, with emphasis on the management of pain and other symptoms.
• Completes ongoing assessments of the patient’s physical status and the psychosocial status of the patient/family.
• Initiates and develops a plan of care in collaboration with the IDG, the patient/family and the physician.

Required Skills

• Graduate of an accredited Registered Nurse (RN) program
• Active, unrestricted Registered Nurse (RN) license in the State of Georgia or compact multi-state license
• Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years.
• One (1) year of hospice care experience preferred.
