St. Croix Hospice in Bellevue, Nebraska is seeking a Registered Nurse on a PRN basis to deliver compassionate hospice care in patients' homes. You will assess needs, administer medications, develop and revise the Plan of Care, and document care provided under the guidance of the RN Case Manager.

You will coordinate with interdisciplinary team members, support families, and ensure comfort and dignity for patients while maintaining HIPAA compliance and professional standards in a home-care setting.

Qualifications

• Graduate of an accredited school of nursing; Associate degree required.
• Current RN license in state(s) of practice.
• Ability to pass background, consumer report and drug test.
• Valid State Driver’s License and access to an automobile.

Responsabilités

• Perform comprehensive patient assessments and develop Plan of Care.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical notes and patient records.
• Coordinate Plan of Care with the interdisciplinary care team.
• Participate in on-call duties and family education.
