Job summary

Amedisys is seeking a Hospice RN Per Diem to join our home health and hospice team in Virginia. You’ll perform initial nursing assessments on new admissions, coordinate and revise the plan of care, and ensure continuity of patient care with the clinical team.

This position offers attractive pay ($40-$42 per hour) plus mileage, on-call duties, and the chance to educate families and support patients at home. Per diem scheduling provides flexibility while contributing to high-quality patient

Qualifications

• Current RN license for the applicable state.
• One year of nursing experience preferred; new graduates eligible.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
