Job summary

Amedisys is seeking a Registered Nurse for Hospice care in Evansville, IN. This full-time, first-shift position offers a $80,000–$85,000 salary with a $10K sign-on bonus and comprehensive benefits.

Responsibilities include initial nursing assessments, coordinating care plans, and communicating patient progress to the care team. Requires current RN license and CPR; valid driver’s license and transportation are required.

Qualifications

• Current RN license is required for the state(s) you are assigned to work.
• One year of nursing experience preferred; new grads considered for RN program.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Performs initial nursing assessment on new admissions.
• Completes, coordinates, and revises the total plan of care and maintains continuity of patient care by collaborating with appropriate staff.
• Regularly communicates patient progress to the clinical manager, physician, and care team.
• Develops, prepares, and maintains individualized patient care progress records with accuracy and timeliness.
• Records pain/symptom management changes/outcomes as appropriate.
• Educates families on patient condition, needs and care.
• Makes referrals to other hospice care disciplines as needed, or documents rationale for not doing so.
• Performs on-call responsibilities and on-call services to patients/families as assigned.
• Performs other duties as assigned.
