Job summary

HopeWest is seeking a Hospice Registered Nurse in Montrose, CO, for a Monday–Friday schedule from 8:00am to 5:00pm. You will deliver compassionate, patient- and family-centered care in home settings, coordinating with an interdisciplinary team to achieve goals of care.

A 10K sign-on bonus is offered. Minimum 1–2 years of nursing experience is preferred; current CO RN license and CPR certification are required.

Qualifications

• 1–2 years of nursing experience preferred.
• Current Colorado RN license maintained throughout employment.
• Current CPR certification maintained throughout employment.
• Valid Colorado driver’s license and reliable transportation.
• Experience with EHR systems and Microsoft Office applications.

Responsibilities

• Provide compassionate, relationship-based care to patients and families.
• Lead the interdisciplinary team in patient care.
• Manage a defined caseload and coordinate home-based services.
• Educate patients and caregivers to meet personal goals of care.
