Job summary

Bristol Hospice in Green Bay, WI is seeking a compassionate Registered Nurse (RN) for PRN, hourly work to support patient and family needs in hospice care. The role requires strong assessment skills and the ability to deliver personalized, compassionate care.

The position offers an hourly wage between $42.00 and $48.00, with flexible shifts and opportunities to contribute to a mission-driven team focused on dignity and comfort at end of life. Travel across the service area may be required.

Qualifications

• Must have at least one year of acute care experience in an institutional setting.
• One year of experience in home care preferred.
• Must have a current licensure in the state.

Responsibilities

• Complete an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness.
• Provide professional nursing care utilizing all elements of the nursing process.
• Assess and evaluate patient status and write/initiate plan of care.
• Regularly re-evaluate patient and family/caregiver needs and revise plans as necessary.
• Initiate and revise the plan of care as patient status changes.
• Use health data to determine nursing diagnoses and goals.
• Include patient and family in planning and education on self-care.
• Record pain/symptom management and communicate with physician.
