Job summary

Qualifications

Responsibilities

• Identify patient and family needs and provide supportive care per physician orders and plan of care.
• Manage nursing process phases (assessing, planning, implementing, evaluating) for hospice patients.
• Coordinate care by communicating with attending physicians, hospice staff, and other agencies.
• Educate caregivers and volunteers to provide appropriate care as indicated.
• Provide 24/7 coverage for patients when necessary and assist at time of death as needed.
