Qualifications

• Graduate of an accredited school of professional nursing and current PA nursing license.
• Two years experience as a professional nurse in nursing practice or community health and/or Hospice care.
• BSN preferred.

Responsibilities

• Determines eligibility for admission; formulates initial care plan according to patient condition.
• Collaborates with the interdisciplinary team to assure appropriate care planning.
• Communicates with the patient’s physician and Hospice Medical Director, both orally and in writing as required.
• Supervises volunteers and provides bereavement care.
• Provides comprehensive nursing care, health teaching, guidance and supervision to patient and family.
• Maintains and uses patient records and reports according to policy; ensures timely documentation.
• Participates in development and operation of the agency, orientation of new staff, and evaluation of services.
