Ascend Hospice in Topeka, KS is seeking a compassionate Hospice Registered Nurse Case Manager (RN) to join our team. You will oversee on-call coverage, patient visits, and care coordination to ensure timely, high-quality hospice services.

Responsibilities include initial assessments, ongoing re-evaluations, developing care plans, counseling families, coordinating with physicians, and supervising Hospice Aides. Must have an RN license and a valid driver’s license.

Qualifications

• Associate's Degree in Nursing.
• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessments of patients and families to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to direct the work of the team to provide continuous and well-coordinated care.
• Counsel patients and families in meeting end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with physicians regarding patients' needs and reporting any changes in patients' conditions.
• Supervise Hospice Aides.

About the Role:

As a Hospice Registered Nurse Case Manager (RN), you will be responsible for assisting the team with on-call, case management, and visits to ensure adequate coverage and call-outs. Your duties will include but are not limited to:
