Ascend Hospice in Overland Park, KS is seeking a Hospice Registered Nurse Case Manager (RN) to join our team. The role supports on-call/case management and patient visits for high-quality end-of-life care. A 5k signing bonus is offered.

You will assess patients, develop care plans, collaborate with the healthcare team, educate families, and supervise aides to ensure coordinated, compassionate care across the continuum.

Qualifications

• Associate's Degree in Nursing required.
• State Certified RN license required.
• 2 years' nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluates the patient’s nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the entire healthcare team, patient and family, to coordinate care.
• Counsels the patient and family in meeting end of life needs.
• Initiates appropriate preventative and palliative nursing procedures.
• Communicates with the physician regarding patient's needs and reports any changes.
• Supervises the Hospice Aides.

Hours: 8:30am - 5:00pm

Ascend Hospice is seeking a dedicated and compassionate Hospice Registered Nurse Case Manager (RN) to join our team in Overland Park, KS. As a valued member of our team, you will play a crucial role in providing high-quality care to patients and their families, while also supporting the overall mission of our organization.

About the Role:

As a Hospice Registered Nurse Case Manager (RN), you will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Your duties will include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides

Requirements:

• Associate's Degree in Nursing
• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills
