Job summary

Ascend Hospice is hiring an RN Case Manager in Lawrence, KS. The role involves on-call and case management with visits, coordinating care across the health team, and supporting patients and families in end-of-life care.

The position follows a M-F 8:30AM-5PM schedule with travel to patient sites as needed. Required: Associate's Degree in Nursing and a State Certified RN license, plus 2 years of professional nursing experience (hospice preferred).

Qualifications

• Strong verbal and written communication skills.
• Must have a State Certified RN license.
• 2 years' experience as a professional nurse, hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.

Responsibilities

• Complete initial assessment of patient and family to determine hospice care needs.
• Regularly re-evaluate patient's nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team to coordinate care.
• Counsel patient and family in end-of-life needs.
• Initiate appropriate nursing procedures.
• Communicate with physician regarding patient needs and changes.
• Supervise the Hospice Aides.

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
