Job summary
Ascend Hospice in Topeka, KS is seeking a compassionate Hospice Registered Nurse Case Manager (RN) to join our team. You will assist with on-call coverage, case management, and visits to ensure comprehensive, well-coordinated care.
Responsibilities include initial assessments, care plan development, and collaboration with physicians, patients, and families. Travel and strong communication skills are essential to support end-of-life care.
Qualifications
- Associate's Degree in Nursing is required.
- Must have a State Certified RN license.
- 2 years' experience as a professional nurse; hospice experience preferred.
Responsibilities
- Perform initial patient and family assessments to determine hospice care needs.
- Regularly re-evaluate patients' nursing needs and adjust care plans.
- Develop and implement care plans using assessment data.
- Collaborate with healthcare team, patient, and family for coordinated care.
- Counsel patients and families in end-of-life needs.
- Initiate appropriate preventive and palliative nursing procedures.
- Communicate changes in patient condition to physicians.
- Supervise Hospice Aides.
