Ascend Hospice in Lawrence, KS is seeking a dedicated RN Case Manager to support on-call coverage, case management, and patient visits. The role requires completing assessments, coordinating care plans, and collaborating with the care team to ensure continuous, well-coordinated services.

Ideal candidates hold an RN license, have 2+ years of nursing experience (hospice preferred), and are willing to travel with reliable transportation.

Qualifications

• Must hold a State Certified RN license.
• 2 years' nursing experience; hospice experience preferred.
• Willingness to travel and valid driver’s license.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial assessments to determine hospice care needs.
• Re-evaluate patient nursing needs regularly.
• Develop care plans from assessment data.
• Collaborate with healthcare team, patient, and family.
• Counsel patients and families on end-of-life needs.
• Initiate appropriate preventativ e and palliative nursing procedures.
• Communicate changes in patient condition to physician.
• Supervise Hospice Aides.

Now Hiring – RN Case Manager – Lawrence, KS

Hours: M-F 8:30AM-5PM

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills.
