Job summary

Ascend Hospice is hiring a full-time RN Case Manager in Liberty, MO to support on-call coverage, case management, and patient visits. You will assess needs, develop care plans, and coordinate with the healthcare team to ensure continuous, high-quality end-of-life care.

Requirements include an Associate's Degree in Nursing, an active RN license, and 2 years of nursing experience (hospice preferred). Travel and valid driver's license are required.

Qualifications

• State-licensed registered nurse with active license.
• Excellent verbal and written communication skills.
• Willingness to travel as required for patient care.

Responsibilities

• Complete initial patient and family assessment to determine hospice needs.
• Regularly re-evaluate patient nursing requirements and adjust care plans.
• Collaborate with healthcare team to coordinate continuous care.
• Counsel patients and families on end-of-life needs and expectations.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate patient status changes with physicians and supervise Hospice Aides.

Now Hiring – RN Case Manager – Liberty, MO

Hours: M-F 8:30am-5pm

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides

Education

Associate's Degree in Nursing

Professional Experience

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills
