Optimal Care - Evergreen is seeking a Hospice RN Case Manager in Michigan to deliver skilled nursing care and compassionate support at a patient’s end of life. You will build close relationships with patients, families, and the care team, coordinating services and ensuring comfort and dignity.

The role involves daily travel within the North Michigan service area, with flexible full-time, part-time, or per diem options and on-site visits from Saginaw, MI.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one year experience as a nurse in a clinical care setting.
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage.

Responsibilities

• Build rapport with patients, families, physicians, facilities, and other team members.
• Coordinate comprehensive care for your caseload in the home or facility setting.
• Travel daily within the service area to provide care to patients in homes, assisted living facilities, or nursing homes.
• Collaborate with the interdisciplinary team to address physical, emotional, and spiritual needs.

Optimal Care is building a talent pipeline of compassionate Registered Nurses for future Hospice RN Case Manager opportunities serving Mid‑Michigan. As a Hospice RN Case Manager, you'll provide skilled nursing care and compassionate support to patients and families during life's final chapter, helping ensure comfort, dignity, and quality of life.

About the Role

This role is perfect for a registered nurse who wants to practice the full scope of nursing while building deep, meaningful relationships. This position will have you building rapport with patients, families, physicians, facilities, and other interdisciplinary team members. You'll have the autonomy to coordinate comprehensive care for your caseload while working collaboratively with our interdisciplinary team to address the physical, emotional, and spiritual needs of those in your care.

Location

• Office Location: Saginaw, MI
• Main Service Area Options:
• North - Crawford / Roscommon / Ogemaw / Iosco / Arenac / Gladwin / Clare / Midland
• This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Full Time: Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Part Time: Three weekdays, 8 a.m. to 5 p.m., with a weekend on‑call rotation
• Per Diem: Flexible weekdays, Monday to Friday, 8 a.m. to 5 p.m., with a weekend on‑call rotation

Required Qualification

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
• Current State License as a Registered Nurse
• Minimum one (1) year experience as a nurse in a clinical care setting
• Reliable transportation, valid driver’s license, and valid automobile insurance coverage

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Hospice or home health experience

• Nursing skills as defined by accepted nursing standards
• Demonstrates ability to assess and respond to the needs of patients, families and caregivers in varied settings and to function effectively in the home or nursing facility environment
• Demonstrates skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
• Ability to handle multiple priorities, documentation requirements and deadlines

Pay Range

$77,000 - $83,000 USD
