Job summary

AccentCare seeks an experienced RN Case Manager to deliver individualized home health care in collaboration with the patient’s physician. You will develop daily care plans, apply nursing interventions, and coordinate with the care team to address complex patient needs.

You will work with relative autonomy in a flexible schedule to meet patient needs, while benefiting from onboarding support and opportunities to grow through our Clinical Ladder program.
