Job summary

AccentCare is seeking an RN Case Manager in Houston to deliver customized one-on-one home health care in collaboration with patients and their physicians.

You will develop daily care plans, apply evidence-based nursing interventions, and exercise autonomy to meet complex patient needs, while benefiting from a structured onboarding and ongoing training that supports your career growth.

Qualifications

• RN license required or must be currently licensed as a Registered Nurse.
• Experience with case management in home health or similar setting.
• Ability to develop patient-centered care plans and coordinate care.

Responsibilities

• Deliver customized one-on-one home health care in collaboration with patients and physicians.
• Develop and implement daily care plans addressing complex patient needs.
• Apply evidence-based nursing interventions and latest clinical practices.
• Exercise autonomy to manage patient schedules and priorities in the field.

You will have the relative autonomy and authority to utilize your extensive experience, training, and critical problem-solving skills.
