As an RN Case Manager, you will work at the top of your licensure, collaborating one-on-one with clients to deliver customized care. Under the guidance of your supervising physician, you will develop care plans and utilize nursing skills and techniques to ensure quality care daily.

As an RN Case Manager, you will:

1. Assess all medications to identify ineffective drug therapy, adverse reactions, side effects, allergies, and contraindications.
2. Establish realistic, measurable, and observable goals aligned with the patient's diagnosis.
3. Comply with legal requirements, standards, policies, and procedures, including the Code of Conduct, HIPAA, and documentation standards.
4. Demonstrate professional conduct, service excellence, and commitment to AccentCare's values and mission.

Requirements for Qualified Candidates:

1. Graduate from an approved nursing program and hold an active RN license in the state of operation.
2. At least one (1) year of experience as an RN.
3. Valid CPR certification while employed in a clinical role.
4. Sufficient endurance to perform tasks over long work hours.
5. Licensed driver with a valid, insured vehicle in good working order, with the ability to travel to all business locations.
