Job summary

Elara Caring in Michigan seeks a dedicated Hospice Registered Nurse Case Manager to join our home-based care team. You will assess needs, monitor patient status, and educate families within an interdisciplinary Plan of Care to ensure comfort and quality.

Required RN licensure in Michigan, 1+ year of clinical nursing experience, good driving record, and willingness to travel to patient homes. Hospice or home health experience is preferred, with strong compassion and communication skills.

Qualifications

• Current State license as a Registered Nurse (RN).
• Graduate of an accredited Nursing program (certificate, diploma, ASN, or BSN).
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and transfer patients up to 50 pounds.
• Valid driver’s license and reliable transportation to travel between patient homes.

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes, concerns, or safety issues to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, assisted living facilities, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.

What is Required?

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred
