Job summary

Compassus in Illinois is recruiting a Hospice Registered Nurse / RN Case Manager to support patients and families in home-based hospice care. You will collaborate with a multidisciplinary team to deliver compassionate, coordinated care that honors patient preferences.

Qualifications

• RN license in the state you work.
• Two years of experience as a registered nurse; hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.

Responsibilities

• Provide patient- and family-centered hospice care in collaboration with the care team.
• Educate caregivers and empower them to participate in care decisions.
• Coordinate and communicate care plans across disciplines to ensure comfort and dignity.
• Support patients in spending meaningful time with loved ones and activities they enjoy.
