Job summary

Pathways Healthcare is seeking a dedicated RN to provide in-home patient care across Manchester and surrounding towns. The role is based in Londonderry, NH, with a service area including several communities, and requires full-time availability Monday through Friday.

You will assess patient status, develop and execute care plans, administer medications, and maintain detailed clinical notes, coordinating with physicians to ensure high-quality care.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records and strong bedside manner.

Responsibilities

• Assess and evaluate patient status and write/initiate care plans.
• Administer medications and treatments as prescribed by the physician.
• Re-evaluate patient and family needs; revise plans as necessary.
• Communicate with physicians and document clinical notes.
• Attend interdisciplinary group meetings and participate in on-call duties.

Position Details:

• Service Area: Manchester, Goffstown, Bedford, Hooksett, Concord, Bow, Suncook, Allenstown, Pembroke areas.
• Job Type: Full-time
• Schedule: Monday – Friday
• Salary Range: $78k-95k/Annual Salary

Responsibilities:

_ Patient Care: _

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

_ Additional Duties: _

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
  

Qualifications:

• 2+ years’ nursing experience, preferably in geriatrics is desirable.
• New nurses, feel free to apply! We have a great training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable, insured vehicle.
