Job summary

Pathways Healthcare in Manchester, NH is seeking an experienced registered nurse for full-time, Monday–Friday in-home patient care. You will assess patients, develop and implement care plans, administer medications, and collaborate with physicians to ensure high-quality outcomes.

Applicants should have 2+ years of nursing experience (geriatrics preferred), a nursing degree, valid BLS/CPR, and an active driver's license.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable vehicle.

Responsibilities

• Assesses and evaluates patient's status; writes and initiates plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Revising the plan of care as necessary.
• Develops a care plan with goals based on nursing diagnosis.
• Initiates preventive and rehabilitative nursing procedures; administers medications.
• Documents and reports patient condition and care given.
• Communicates with physician regarding patient needs and changes.
• Maintains a safe patient environment and attends meetings.
• Participates in on-call duties and coordinates equipment/services.
• Supervises ancillary personnel and delegates as needed.

Position Details:

Responsibilities:

_ Patient Care: _

• Assesses and evaluates patient's status by:


• Writing and initiating plan of care.


• Regularly re-evaluating patient and family/caregiver needs.


• Participating in revising the plan of care as necessary.


• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.


• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.


• Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.


• Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.


• Provides and maintains a safe environment for the patient.


• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.


• Ensures that arrangements for equipment and other necessary items and services are available.


• Supervises ancillary personnel and delegates responsibilities when required.


• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.


• Fulfills the obligation of requested and/or accepted case assignments.


• Actively participates in quality assessment performance improvement teams and activities.
  

Qualifications:

• 2+ years' nursing experience, preferably in geriatrics is desirable.
