The ideal candidate has 2+ years of nursing experience, an accredited nursing degree, and current BLS CPR certification. A valid driver's license and reliable vehicle are required.

Qualifications

• 2+ years of nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

• Assesses and evaluates patient status and writes plan of care.
• Regularly re-evaluates patient and family needs and revises plan.
• Develops care plans with goals based on nursing diagnosis.
• Administers medications and treatments as prescribed.
• Maintains accurate clinical notes and reports changes in condition.
• Communicates with physicians and orders as required.
• Provides a safe environment and attends interdisciplinary meetings.
• Participates in on-call duties and ensures equipment and services are available.

Position Details:

• Main Office location: Londonderry, NH
• Service Area: Salem, Atkinson, Plaistow, Kingston, Danville, East Pelham, Windham, Sandown areas.
• Job Type: Full-time
• Schedule: Monday – Friday
• Salary Range: $78k-95k/Annual Salary

Responsibilities:

_ Patient Care:_

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

_ Additional Duties:_

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
