Job summary

Pathways Healthcare seeks an experienced nurse to provide in-home care across Fall River and surrounding areas with a strong geriatrics focus. The role operates from our Westwood, MA main office and covers a full-time, Monday–Friday schedule including on-call duties.

The ideal candidate holds a nursing degree, is proficient with EMR, maintains BLS CPR, and carries a valid driver’s license. You will collaborate with physicians to implement plans of care, document patient progress, and support

Qualifications

• 2+ years of nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver’s license and own a reliable vehicle.

Responsibilities

• Assess and evaluate patient status and write/initiate plan of care.
• Regularly re-evaluate patient and family needs and adjust the plan.
• Administer medications and treatments as prescribed.
• Maintain accurate clinical notes and record changes in pain/symptom management.
• Communicate with physicians and obtain orders as required.
• Provide a safe home environment and attend interdisciplinary meetings.
• Participate in on-call duties and ensure equipment readiness.
• Support quality improvement initiatives.

Position Details:

• Main Office location: Westwood, MA
• Service Area: Fall River and Surrounding Areas
• Job Type: Full-time
• Schedule: Monday – Friday

Responsibilities:

Patient Care:

• Assesses and evaluates patient’s status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
