Redcrest Hospice LLC in Bloomington, IN is seeking a Registered Nurse (RN) I - Hospice to provide and plan professional nursing care under a physician's direction. This role delivers direct patient care in patients' homes, with duties including documenting conditions, vital signs, diagnoses, and prescribed treatments.

The position features a full-time schedule Monday through Friday, 8:00 AM to 4:30 PM, with holiday rotation and no weekends or on-call duties.

Qualifications

• 0 to 2 years of experience.
• Valid RN license in the state(s) where service is provided.
• Bachelor's degree in nursing or healthcare-related field preferred.

Responsibilities

• Provides skilled nursing care in a patient’s home as ordered by the attending physician.
• Observes and monitors patient conditions
• Performs assessments to develop an individualized plan of care and adjusts as needs change
• Administers medication as prescribed by the physician
• Manages multi-disciplinary care as applicable
• Promotes continuity of care with appropriate admissions, transfers and discharges
• Counsels’ patient and family on the disease/injury processes and how to manage
• Incorporates patient and family in development of plan of care
• Oversees and supervises total care of patient provided by Aides and LPNs
• Evaluates the treatments and medical condition of the patient according to the plan of care
• Other duties as assigned

Education

RN license in state(s) where service is provided
