Job summary

Angels Care Hospice in Texas is seeking a dedicated RN Case Manager to coordinate patient home care. You will work with the medical team to ensure high-quality, patient-centered services.

This role covers admission visits, eligibility assessments, and developing and monitoring the care plan. We offer competitive pay, mileage and on-call pay, PTO, and paths for professional growth.

Qualifications

• Graduate of an accredited nursing program (diploma/AD/BSN)
• Current state RN license
• Current state driver’s license
• One-year RN experience, two preferred
• CPR certification and Hepatitis consent/declination
• Reliable transportation with auto liability insurance

Responsibilities

• Coordinate all aspects of the patient’s home care.
• Perform the admission visit to the patient in the home and determine eligibility for home care services.
• Develop and follow the plan of care with the patient and medical team.

About the position:

The Registered Nurse (RN) Case Manager coordinates all aspects of the patient’s home care. You will interact with other medical disciplines as needed & provide professional care to the patient. You will also perform the admission visit to the patient in the home, determine the patient’s eligibility for home care services, & develop the plan of care to be followed.

Qualifications:

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One-year’s experience as a Registered Nurse, two preferred.
• Proof of current CPR and Hepatitis consent/declination.
• Reliable transportation with valid and current auto liability insurance.
