Job summary

Pathways Healthcare in Westwood, MA is hiring a registered nurse to provide in-home patient care across the Taunton, Attleboro, and surrounding communities. The role focuses on assessment, planning, and implementing individualized care plans with family involvement.

You will work a Monday–Friday schedule, with on-call duties and opportunities for leadership development. A nursing degree, current BLS CPR, and a valid driver's license are required.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics is desirable.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Assesses and evaluates patient's status by writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan based on nursing diagnosis and patient/family input.
• Initiates preventive and rehabilitative nursing procedures and administers medications as prescribed.
• Completes and submits clinical notes and reports changes in patient condition.
• Communicates with the physician regarding patient needs and orders.
• Ensures safe environment and participates in interdisciplinary meetings.
• Participates in on-call duties and ensures necessary equipment and services are available.

Responsibilities:

Patient Care:

• Assesses and evaluates patient's status by:
• Writing and initiating plan of care.
• Regularly re-evaluating patient and family/caregiver needs.
• Participating in revising the plan of care as necessary.
• Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
• Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
• Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
• Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
• Provides and maintains a safe environment for the patient.
• Attends interdisciplinary group meetings.

Additional Duties:

• Participates in on-call duties.
• Ensures that arrangements for equipment and other necessary items and services are available.
• Supervises ancillary personnel and delegates responsibilities when required.
• Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
• Fulfills the obligation of requested and/or accepted case assignments.
• Actively participates in quality assessment performance improvement teams and activities.
