Job summary

Pathways Healthcare in Westwood, MA is seeking a full-time registered nurse to deliver in-home nursing and hospice support. This role follows a Monday–Friday schedule, collaborates with physicians and interdisciplinary teams, and focuses on geriatric home care with rigorous care planning.

You will assess patients, administer treatments, and supervise aides while ensuring compliance with healthcare regulations and maintaining BLS CPR certification.

Qualifications

• Active Nursing license in the state where the position is based.
• Preferably 2+ years of nursing experience with a geriatrics focus.
• New nurses and LPNs are encouraged to apply; training program offered.
• Nursing degree from an accredited institution.
• Excellent bedside manners and communication skills.
• Knowledge of healthcare laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiate preventive and rehabilitative nursing procedures.
• Administer medications and prescribed treatments.
• Conduct initial assessments of patients and families for home care needs.
• Perform thorough physical assessments and gather medical histories.
• Develop and revise individualized care plans.
• Collaborate with physicians, nurse practitioners, and teams on Plans of Care.
• Address inquiries and concerns from patients, caregivers, and medical teams.
• Provide counseling and support before, during, and after treatment.
• Supervise home health aides as required.
• Identify discharge planning needs and implement them in the care plan.

Responsibilities

Compensation & Benefits

• Salary Range: $75,000 - $100,000
