Job summary

Bristol Hospice is seeking a Care Consultant (Hospice Liaison) in Appleton, WI. This full-time role focuses on building relationships with referral sources to connect patients and families with high-quality end-of-life care.

The ideal candidate will travel within the Bristol service area, engage in community outreach, and present to healthcare providers, facilities, and organizations. Competitive salary with incentive bonuses and extensive benefits are offered.

Qualifications

• 1+ year of sales, marketing, professional relations, and/or education training is required
• Experience in hospice sales, home health sales, medical device sales, clinical liaison, RN liaison, clinical coordination or medical sales is preferred
• Ability to communicate with the general public and represent Bristol in a positive manner

Responsibilities

• Build relationships with referral sources in assisted living facilities, hospitals, physician offices, personal care homes, and other community settings
• Be in the community daily, acting as the face of Bristol Hospice
• Deliver outreach calls and presentations to current and prospective referral sources
• Collaborate with clinical and operational teams to ensure seamless care and promote clinical excellence
• Provide patients and families with information about hospice care
• Meet with community partners regarding hospice services
• Participate in regular team calls and maintain trust with community partners
• Drive growth through advocacy with referral sources
