Job summary

ViaQuest Hospice is seeking a Hospice Community Liaison to build and sustain strong referral partnerships with hospitals, skilled nursing facilities, and other sources in the Washington, PA area. You will educate healthcare professionals, families, and the community about hospice services and early referrals, while meeting referral and admission goals.

Ideal candidates bring at least 2 years in sales or business development, with strong relationship-building and CRM skills.

Qualifications

Minimum 2 years of sales or business development experience.
Healthcare or hospice experience preferred but not required.
Strong communication and relationship-building skills.
Ability to manage a full schedule and CRM follow-ups.

Responsibilities

Develop and maintain strong referral relationships with hospitals, SNFs, assisted living communities, and other sources.
Educate providers and families about hospice services, eligibility criteria, and early referrals.
Meet or exceed referral and admission goals with ethical sales practices.
Document and track sales activities, referrals, and follow-ups in a CRM; participate in patient/family follow-up to ensure quality service.
