Job summary

Heritage Hospice in Albuquerque is seeking a Marketing & Education Liaison to strengthen referral relationships and raise awareness of hospice services across the region. This field-based role focuses on education, outreach, and timely access to end-of-life care.

The ideal candidate communicates effectively with clinicians and community audiences, and is passionate about helping patients and families understand hospice benefits. Travel and driving are required to engage partners and communities.

Qualifications

• Experience in hospice, home health, palliative care, senior living, or healthcare outreach preferred.
• Excellent communication, presentation, and interpersonal skills.
• Ability to explain clinical concepts clearly to medical professionals and community audiences.

Responsibilities

• Build and maintain partnerships with hospitals, physicians, SNFs, home health agencies, and referral sources.
• Deliver clear education about hospice services, eligibility, and referral processes.
• Conduct on-site and community presentations, in-services, and outreach events.
• Serve as a resource for healthcare professionals on hospice appropriateness and care transitions.
• Collaborate with intake, clinical leadership, and business development to support referrals and admissions.
• Identify barriers to hospice utilization and develop targeted educational strategies.
• Track referral activities and follow-ups using CRM or organizational tools.
• Represent the organization at community events and professional gatherings.
• Stay current on hospice regulations and Medicare benefit expectations.
