Job summary

Community Healthcare of Texas is seeking a Hospice Quality Improvement Coordinator (In-house) to lead clinical staff education, ensure HQRP compliance, and coordinate quality initiatives across palliative care and hospice services. The role emphasizes data-driven improvement, infection control alignment, and collaboration with executive leadership.

The ideal candidate holds a nursing degree and Texas licensure (RN/LVN), with 5+ years of nursing experience and hospice leadership experience

Qualifications

• Requires nursing degree and Texas RN or LVN licensure.
• Experience in hospice quality improvement and staff education.
• Minimum 5 years of nursing experience.
• Knowledge of hospice regulations and infection control practices.
• 3+ years hospice leadership preferred.

Responsibilities

• Conduct needs assessment and tailor training curricula.
• Monitor regulatory requirements and integrate into education.
• Develop curriculum, presentations and conduct training.
• Coordinate staff orientation and follow-up.
• Lead HQRP data collection, analysis and reporting.
• Support compliance initiatives and QA activities.
• Maintain knowledge base of Home Care Home Base software to produce reports.
• Collaborate with VP Quality & Compliance and execs on policy updates.
• Other duties as assigned.
