The Welcome Process: Cheerfully overseeing the accurate and compliant processing of all Referral Intake paperwork, setting the stage for smooth, compassionate patient admissions.
The Compliance Compass: Conducting thorough and fair chart audits to ensure we proudly meet all regulatory standards, including the essential Conditions of Participation (CoPs).
Medication Safety Superhero: Monitoring and ensuring impeccable Medication Reconciliation processes during care transitions, keeping our patients safe and sound.
HOPE Navigator: Guiding our use of the HOPE program data to discover successes and target improvements that elevate patient outcomes and experience.
Mentor and Educator: Providing positive, engaging training and mentorship to our clinical staff, helping every team member shine bright in their documentation and clinical practice.
New Hire Welcome Wagon: Conducting comprehensive, supportive quality and compliance training for all new hires, ensuring they start their journey with confidence.
QAPI Committee Chair: Leading our QAPI Committee with enthusiasm, turning data into dynamic, meaningful action plans that foster a culture of continuous learning and improvement.
Heart and Skill: Current Registered Nurse (RN) license in the state of Ohio with active CPR card.
Experience That Matters: Minimum of 5 years of clinical hospice experience.
Regulatory Whiz: Deep, current knowledge of hospice regulations, Medicare CoPs, and QAPI methodologies—you know the rules and how to teach them!
Communication Star: Exceptional ability to communicate clearly, listen actively, and build supportive relationships across all departments.
