Job summary

Continuum Care is seeking a Hospice Quality Assurance (QA) Coordinator to lead compliance and quality improvement initiatives that support comfort, dignity, and peace for every family served.

You will mentor staff, oversee regulatory chart audits, and drive improvements using the HOPE program data. The role requires strong communication and leadership in a hospice setting to elevate patient outcomes and experience.

Qualifications

• Must be a Current RN licensed in Ohio with active CPR card.
• Minimum of 5 years of clinical hospice experience.
• Deep knowledge of hospice regulations, Medicare CoPs, and QAPI methodologies.
• Exceptional communication and cross-department relationship-building skills.

Responsibilities

• Oversee Referral Intake processing to ensure accurate, compliant admissions.
• Conduct chart audits to ensure compliance with CoPs and regulatory standards.
• Monitor and safeguard Medication Reconciliation during care transitions.
• Utilize HOPE program data to identify improvements and improve patient outcomes.
• Provide training and mentorship to the clinical staff on documentation and practice.
