Job summary

NorthStar Home Hospice LLC is seeking a dedicated Hospice Quality Assurance and Performance Improvement professional to join our Cincinnati care team, focusing on regulatory compliance and clinical excellence in end-of-life care.

You will lead the QAPI program, analyze clinical and operational data, conduct audits, and collaborate with interdisciplinary staff to drive performance improvements and ensure CMS Conditions of Participation are met.

Qualifications

• RN licensure in Ohio required.
• Experience in hospice, home health, or palliative care QA or compliance.
• Knowledge of CMS hospice CoP and QAPI regulations.
• Experience with clinical auditing and data analysis.
• Strong organizational, communication, and critical-thinking skills.
• Proficiency with EHR and quality reporting tools.

Responsibilities

• Develop, implement, and oversee the QAPI program in compliance with hospice regulations and standards
• Monitor and analyze clinical and operational data to identify trends, gaps, and opportunities for improvement
• Lead performance improvement initiatives and coordinate interdisciplinary team involvement in quality projects

• Prepare and present quality reports and metrics to leadership and clinical staff

Responsibilities:

• Prior management experience in hospice, home health or pallaitive care a plus


• Strong knowledge of CMS hospice Conditions of Participation and QAPI regulations


• Experience with clinical auditing, data analysis, and performance improvement methodologies


• Excellent organizational, communication, and critical thinking skills


• Proficiency with electronic health records (EHR) and quality reporting tools
