NorthStar Home Hospice LLC in Cincinnati, OH is seeking a dedicated Hospice Quality Assurance & Performance Improvement professional to lead QAPI efforts, ensure CMS COP compliance, and drive data‑driven improvements across care operations.

The role collaborates with clinicians, audits patient records, develops education, and reports metrics to leadership to uphold compassionate, high‑quality end‑of‑life care.

Qualifications

• Current RN or clinical licensure in Ohio required.
• Experience in hospice, home health, or palliative care quality assurance or compliance.
• Strong knowledge of CMS Hospice Conditions of Participation and QAPI regulations.
• Experience with clinical auditing, data analysis, and performance improvement methodologies.
• Excellent organizational, communication, and critical thinking skills.
• Proficiency with EHRs and quality reporting tools.
• Compassionate, detail-oriented, and committed to high-quality end-of-life care.

Responsibilities

• Develop, implement, and oversee the QAPI program in compliance with hospice regulations.
• Monitor clinical and operational data to identify trends and improvement opportunities.
• Lead performance improvement initiatives and coordinate interdisciplinary involvement.
• Audit clinical documentation, patient records, and care plans for regulatory compliance.
• Prepare and present quality reports and metrics to leadership and staff.

NorthStar Home Hospice LLC is looking for a dedicated and detail-oriented Hospice Quality Assurance & Performance Improvement professional to join our compassionate care team in Cincinnati, OH. Responsibilities:

• Develop, implement, and oversee the Quality Assurance and Performance Improvement (QAPI) program in compliance with hospice regulations and standards
• Monitor and analyze clinical and operational data to identify trends, gaps, and opportunities for improvement
• Lead performance improvement initiatives and coordinate interdisciplinary team involvement in quality projects
• Conduct audits of clinical documentation, patient records, and care plans to ensure regulatory compliance
• Prepare and present quality reports and metrics to leadership and clinical staff
• Ensure adherence to CMS Conditions of Participation and state hospice regulations
• Collaborate with clinical staff to develop and deliver education related to quality standards and best practices

Requirements:

• Current RN or clinical licensure in the state of Ohio required
• Prior experience in hospice, home health, or palliative care quality assurance or compliance
• Prior management experience in hospice, home health or pallaitive care a plus
• Strong knowledge of CMS hospice Conditions of Participation and QAPI regulations
• Experience with clinical auditing, data analysis, and performance improvement methodologies
• Excellent organizational, communication, and critical thinking skills
• Proficiency with electronic health records (EHR) and quality reporting tools
• Compassionate, detail-oriented, and committed to delivering exceptional end-of-life care
