Job summary

Mission Hospice and Home Care in the San Francisco Peninsula and South Bay is growing its hospice sales team. We are seeking a Hospice Provider Relations RN Liaison with hospice experience, positivity, and a growth mindset to educate hospitals, facilities, and providers about the benefits of community-focused not-for-profit hospice.

This full-time role focuses on developing relationships, facilitating admissions, and contributing to a collaborative team.

Qualifications

AA or BSN in Nursing from an accredited Nursing School.
Minimum of one year experience in a home health/hospice setting.
Current license as a registered professional nurse in the State of California.
Public Health Nursing certification desirable; HPNA certification preferred.

Responsibilities

Develop relationships, maintain regular contact, and resolve issues with referral sources to ensure ongoing quality of care and referrals.
Determine patient’s eligibility for hospice care.
Admit patients and inform patient/family of rights and responsibilities.
Assist with discharge coordination from hospitals into the agency’s hospice program.
Communicate with Intake and Admissions regarding hospice referral/admission.
Assist Director of Provider Relations in negotiating or redefining contract provisions as necessary.
