Job summary

Continuum Hospice is seeking a Program Director to oversee the San Jose hospice program, directing day-to-day operations, growth initiatives, and quality care delivery. The role focuses on external market relationships, financial management, customer service, and compliance with Medicare/Medicaid rules.

The position offers a $10,000 sign-on bonus and a Monday–Friday schedule from 8:30 am to 4:30 pm, with opportunities for professional growth and a comprehensive benefits package.

Qualifications

• Bachelor's degree in nursing, health care, or business admin preferred.
• Minimum of two years in health care management, home health, hospice, or related programs.
• Two years of hospice experience with knowledge of Medicare/Medicaid and aging services regs.
• Operations or sales leadership experience required.
• Ability to develop and maintain relationships with communities and staff.
• Experience analyzing processes and managing priorities in a fast-paced environment.

Responsibilities

• Forge strong connections with physicians, hospitals, nursing facilities, and clinics.
• Build the dream team through staffing, orientation, training and evaluation.
• Be the backbone of excellence by organizing administrative functions.
• Maintain compliance with applicable laws and regulations and implement corrective action.
• Lead day-to-day management and operations of the hospice program.
• Implement policies and procedures approved by the governing body.
